Home Health Care

Cass County Public Health Services is a Medicare Certified Home Health Agency, having staff available to provide skilled nursing services, home health aide personal care services, and physical therapy for you in your home.

Other home care support services available to citizens include homemaker services, Senior Companions, hospice volunteer services, respite care, and personal care assistant services.

We can offer:

• Care at home during recovery from an acute illness or injury.
• Care at home for chronically ill/disabled children and adults.
• Care at home for the terminally ill.
• Help adjusting to an illness for disability.
• Information about a new diet, medication, or disease.
• Personal care and support services to help you stay in your home as long as possible.
• Periodic visits by a public health nurse to check on you in your home.
• Information about other community resources.
• Access to medical supplies and equipment.

LongTerm Care Consultation: Prior to nursing home placement, an assessment is done by a Public Health Nurse and Social Worker to determine a person's health and social needs. This screening is an excellent resource for families unsure of what to do with an aging person who needs care.
